Caregiver Support in Action

Healthy Seniors of Steele County collaborates with staff, nurses, social workers from Allina Health System, Mayo Health Clinic of Owatonna, and Koda Living Community.  We deal with a variety of requests from talking with seniors and caregivers that are transitioning home.

John's Story

John was 98 years old and was in short term care at Koda Living Community after he had taken his second fall since November. Koda Living Community made the referral to Healthy Seniors of Steele County. We met with John’s daughter, she is old enough to retire and when we met, she was in tears and under a high level of stress with Jury duty and her position. She was contemplating on retiring as she did not like the options available to her father in wanting to live in their home. John has limited vision and is partially blind. John was in need of respite care, and was given exercises to help reduce his risk of falling as part of his care plan once he was able to return home.  Healthy Seniors coordinated staff and volunteers to provide respite care and exercises each week to support his daughter, who is his primary caregiver. This support helped her continue to stay in her career, and helped John continue to live in his home.
